DescriptionCBT-I (Cognitive Behavioral Therapy for Insomnia) is currently considered the first-line therapy for persistent insomnia. This suggestion, which was published in the Annals of Internal Medicine (2016;165(2):125-133), was issued by the second biggest medical organization in the United States (The ACP).
The advice is based in part on the following results.
In the short term, CBT-I is equally efficacious and effective as medication, but it is more long-lasting.
CBT-I has fewer adverse effects than medication.
CBT-I improves sleep continuity significantly and does so generally regardless of medical or mental comorbidities.
CBT-I directly addresses the variables hypothesized to promote insomnia (i.e., the mismatch between sleep opportunity and sleep ability).
CBT-I has been demonstrated to be effective even in patients who have a lengthy history of taking hypnotics.The Penn Basic CBT-I Course is a two-day comprehensive study of the intervention’s concepts and practice. The 16-hour training, combined with supervision of practicum cases (peer consultation from credentialed BSM individuals [or established experts]) and targeted readings (Tx manuals and practice papers), should position established clinicians to 1) deliver CBT-I reliably and effectively and 2) be eligible for the credentialing exam being developed by the SBSM and related agencies. Donn Posner, Ph.D., a Diplomate in Behavioral Sleep Medicine (DBSM), works for the Palo Alto VA as a clinical/research psychologist.
Prior to joining the VA, he was a clinical associate professor at Brown Medical School for 25 years. He was the director of clinical behavioral medicine at Rhode Island and Miriam hospitals, as well as the director of behavioral sleep medicine for Lifespan Hospitals’ Sleep Disorders Center. Dr. Posner was the principal supervisor for a rotation of the behavioral medicine track of the clinical psychology internship at Brown for 20 of those years. He also coached postdoctoral fellows and taught interns, fellows, and residents in internal medicine and psychiatry on behavioral sleep medicine and anxiety disorders. In addition, he served as a consultant for the Veteran’s Administration’s adoption of CBT-I and taught VA physicians around the country in its use.
Dr. Posner is a co-author of Cognitive Behavioral Treatment of Insomnia: A Session-by-Session Guide (Springer/Verlag, New York). The book is aimed for clinical trainees and non-insomnia sleep specialists, as well as more experienced clinicians from other fields who want to learn how to deliver scientifically supported cognitive behavioral treatment for insomnia (CBT-I).
Dr. Posner is a member of the American Academy of Sleep Medicine and was one of the organization’s first certified behavioral sleep medicine experts. He is also a founding member of the Society of Behavioral Sleep Medicine and has acquired the title of Diplomate with the SBSM, the organization’s highest degree of qualification and proficiency.

Associate Professor at the University of Pennsylvania Perelman School of Medicine and Director of the Behavioral Sleep Medicine ProgramMichael L. Perlis, PhD is well-known globally for his work in the field of Behavioral Sleep Medicine (BSM). He is the senior author of a published CBT-I treatment manual and a bigger text covering all BSM therapies, as well as a coauthor of the first text book in this subject. Neurocognitive phenomena in insomnia, the cognitive and/or behavioral effects of sedative hypnotics and placebos, the creation of alternative therapeutic techniques for insomnia, and sleep in depression are among his research interests. His research has been and continues to be supported by the National Institutes of Health, and he has produced over 100 articles and chapters on sleep-related issues.
Aside from his academic pursuits, he has served on the editorial boards of Sleep, the Journal of Sleep Research, Sleep Medicine Research, and Behavioral Sleep Medicine. Dr. Perlis has also served on or chaired various committees and task forces of the Sleep Research Society and the American Academy of Sleep Medicine, and he was a founding member and first president of the Society of Behavioral Sleep Medicine (2010-2011).

Jason Ellis, PhD is a sleep science professor and the director of the Northumbria Centre for Sleep Research in the United Kingdom. He divides his time between his fundamental research interests in sleep disorders pathophysiology (Insomnia, Restless Legs Syndrome, and Circadian Rhythm Disorders), the natural history of Insomnia, and his applied work on Cognitive Behavioral Therapy for Insomnia (CBT-I). He investigates the impact of innovative adjunct treatments, the impact of social variables on adherence, and the successful delivery of CBT-I in difficult patients within the latter framework. He has worked in the United Kingdom’s National Health Service, administering CBT-I to people with a variety of medical and psychological disorders, and he is on the editorial board of Behavioral Sleep Medicine. He is a strong supporter of public participation and professional education in behavioral sleep medicine, and he routinely teaches clinicians and primary care physicians in the United Kingdom in CBT-I.
